* CORDA-3194 Do not allow killed flows back into the hospital
This change has been made to prevent killed flows from being added back
to the hospital after being forcibly removed by `killFlow`. Not doing so,
could leave references to a flow inside of the hospital, which is not
the correct behaviour.
`killFlow` now sets a flow's `StatemachineState.isRemoved` to true.
This check is then used in `StaffedFlowHospital` and the
`DumpHistoryOnErrorInterceptor`.
* CORDA-3194 Log different message for transition error due to killed flow
When a flow is killed, its checkpoint is deleted. Currently, the
statemachine will still try a process the next event even if it has
been killed. This can lead to an error when trying to update the
deleted checkpoint. The exception thrown from this is logged out.
An if statement has been added to log a different message at debug level
if it is due to an update error for a killed flow. This is done to not
alarm node operators of the exception.

* CORDA-3194 Replay start flow events when responding flow fails initial checkpoint commit
Logic has already been added to recover from initial checkpoint commit
failures on the initiating flow side but this did not suffice for
the same failure occurring on the responding flow's side.
The same idea has been added to resolve the responding flow's issue.
`ExternalMessageEvent` now has a `flowId` that is maintained on the
event. Messages can then be replayed to start/restart the flow, while
the event provides the flow id to each flow start.
Each `ExternalMessageEvent` implementation generates a random `flowId`
when constructed.
Events are stored in Artemis. This allows the solution to recover across
node restarts as the events will be pulled from artemis again when
restarting.
In the future `flowId`s will probably moved off of the events and
generated purely on the responding flow's node.

Wrap exceptions that occur in state machine transitions with a custom exception type which is
then handled inside of the flow hospital. As part of this change, a number of side negative side
effects have been addressed.
General summary:
- `StateTransitionException` wraps exceptions caught in `TransitionExecutorImpl`
- `StateTransitionExceptions` are handled in the flow hospital, retried 3 times and then kept in
for observation if errors persist (assuming conditions below are false)
- Exceptions that occur in `FlowAsyncOperation` events are wrapped in
`AsyncOperationTransitionException` and ignored by the flow hospital transition staff member
- `InterruptException`s are given a `TERMINAL` diagnosis by the flow hospital transition staff
member (can occur due to `killFlow`)
- Allow flows which have not persisted their original checkpoint to still retry by replaying their
start flow messages
- Swallow exceptions in `AcknowledgeMessages` actions
Detailed summary:
* CORDA-3194 Add state machine transition error handling to flow hospital
Wrap exceptions that are caught in `TransitionExecutorImpl` (coming from
new errors) with `StateTransitionException`. This exception is then
handled explicitly by the flow hospital.
Add `TransitionErrorGeneralPractitioner` to `StaffedFlowHospital`. This
staff member handles errors that mention `StateTransitionException`.
Errors are retried and then kept in the hospital if the errors persist.
* CORDA-3194 Remove a fiber from the `hospitalisedFlows` if its previous state was clean
If the fiber's previous state was clean then remove it from
`HospitalisingInterceptor.hospitalisedFlows`. This allows flows that are
being retried to clean themselves. Doing this allows them to re-enter
the flow hospital after executing the fiber's transition (if an error
occurs).
This is important for retrying a flow that has errored during a
transition.
* CORDA-3194 Set `isAnyCheckpointPersisted` to true when retrying a flow
Added to prevent a single flow from creating multiple checkpoints when
a failure occurs during `Action.AcknowledgeMessages`.
More specifically, to `isAnyCheckpointPersisted` is false when retrying
the flow, even though a checkpoint has actually been saved. Due to this
a brand new flow is started with a new flow id (causing duplication).
Setting `isAnyCheckpointPersisted` to true specifically when retrying a
flow resolves this issue.

Add `StatemachineErrorHandlingTest` to verify transition error handling.
Byteman allows exceptions to be injected at certain points in the code's
execution. Therefore exceptions can be thrown when needed inside of the
state machine.
The current tests check errors in events:

- `AcknowledgeMessages`
* CORDA-3194 Swallow all exceptions in `ActionExecutorImpl.executeAcknowledgeMessages`
Swallow the exceptions that occur in the `DeduplicationHandler`s when
inside of `ActionExecutorImpl.executeAcknowledgeMessages`.
The side effects of the failures that can happen in the handlers are
not serious enough to put the transition into a failure state.
Therefore they are now caught. This allows the transition to continue
as normal, even if an error occurs in one any of the handlers.
* CORDA-3194 Wrap unexpected exceptions thrown in async operation transitions
Exceptions thrown inside of `FlowAsyncOperation.execute` implementations
that are not returned as part of the future, are caught, wrapped and
rethrown. This prevents unexpected exceptions thrown by (most likely)
user code from being handled by the hospital by the transition
staff member.
This handling might change moving forward, but it allows the async
operation to continue working as it was before transition error handling
was added.
* CORDA-3194 Verify that errors inside of `AcknowledgeMessages` work as expected
Update `StatemachineErrorHandlingTest` to correctly test errors that
occur when executing the `AcknowledgeMessages` action.
* CORDA-3194 Retry flows that failed to persist their original checkpoint
Allow a flow that failed when creating their original checkpoint (for
example - failing to commit the db transaction) to retry.
The flow will create a brand new checkpoint (as the original did not
saved).
This required adding `flowId` to `ExternalStartFlowEvent` to allow the
event to keep a record of the flow's id. When the flow is retried, the
events are replayed which trigger a flow to be started that has the
id stored in the event.
To allow this change, code was removed from `retryFlowFromSafePoint` to
allow the function to continue, even if no checkpoint matches the passed
in flow id.
* CORDA-3194 Correct `FlowFrameworkTests` test due to error handling
Test assumed that errors in transitions are not retried, this has now
been updated so the test passes with the flow succeeding after an
exception is thrown.

NotaryFlow.Client flow assumes the passed transaction is verified, which is
the case when it's invoked as a sub-flow in FinalityFlow. However,

* CORDA-3904 Add `flowCallStackSummary`
Add `flowCallStackSummary` to the output which contains the same content
as `flowCallStack` minus each subflow's `FlowLogic`. The `FlowLogic`
contains a ton of info which is normally repeated in each subflow.
Adding the summary gives an overview of the steps the flow executed
and which step it is currently on.
The `suspendedOn` field is put underneath the summary and the original
call stack is moved below the suspended info.
This puts the most useful information towards the top of the json file.

`TransactionUtils.isAttachmentTrusted` requirements have been relaxed
to allow an untrusted attachment to be loaded as long as another
attachment exists that is signed by the same keys and was uploaded
by a trusted uploader.
The requirement of containing the same contract classes has been
removed. Therefore the contents of the existing trusted attachment
no longer matters.
* CORDA-3018 Allow a subset/intersection of signers in `isAttachmentTrusted`
Allow a subset/intersection of signers to satisfy the signer
requirements of `isAttachmentTrusted`. This allows an "untrusted"
attachment that is signed by one or more keys to be "trusted" as long
as another trusted attachment already exists that is signed by at least
one of the "untrusted" attachments signers.
A cache of trusted and untrusted public keys is now held (replacing the
previous cache of `List<PublicKey>`.
Tests have been added to `NodeAttachmentServiceTest` to confirm that
an attachment query using an `EQUAL` statement will actually return
attachments that are signed by any of the keys passed into the query.
Confirming this allowed an `EQUAL` query to satisfy the search that
had to be done as part of this change.
`MockAttachmentStorage`'s query criteria was updated to better match
the real `NodeAttachmentService` implementation.

* ENT-3801: Store downloaded txns as part of the backchain resolution into the db rather than the checkpoint
It's very inefficient to store the downloaded backchain in the checkpoint as more of it downloaded. Instead, if a threshold is reached (which currently defaults at 0) then the backchain is stored in the transactions table as unverified. A new is_verified column has been added to track this. Initially testing on the OS codebase has been very promising but unfortunately this current code is not quite ready. I had to quickly port it to ENT as this is meant to be an ENT-only optimisation.
To that effect, there is a TransactionResolver abstraction with two implementations: an in-memory one which has the old behaviour, and which will be the behaviour for OS, and a db one.
DBTransactionStorage hasn't been fully updated and I had to comment out the optimistic path for now.
Most of these changes will need to be ported to OS to keep the merge conflicts in check, but obviously not DbTransactionsResolver and the "is_verified" changes in DBTransactionStorage. DBTransactionStorage does have other refactoring which will make sense to port though.

Allow Hash Constraints to propagate to Signature Constraints. When
propagating, the new jar is added to the transaction instead of the
original contract attachment (jar).
The following requirements must be met to do so:
- System property "net.corda.node.disableHashConstraints" must be set
to `true`
- The min platform version must be 4 or greater
- There must be an input state with a hash constraint
- There must be no output states with a hash constraint
- The new jar must be signed
If these requirements are not met, the original Hash
Constraint will be kept and the contract attachment related to it is
also used.
This transition is done at the start of `handleContract` as it is not
the normal path a transition would follow. It is considered a backdoor
and should be treated separately from the rest of the attachment and
constraint logic. Furthermore, it will only work in private network
since all nodes must set the special `disableHashConstraints` flag.

Party and AnonymousParty have been retrofitted to implement this interface and are currently the only supported types. A new FlowLogic.initiateFlow(Destination) method overload to easily support the addition of new destination types in future versions.

interface, `InternalCordaRPCOps` has been created and the function
has been moved there. `InternalCordaRPCOps` inherits from
`CordaRPCOps`.

changes due to issues handling inherited functions. This has only been

When a `UnexpectedFlowEndException` or a `FlowException` is received the
peer that the exception was thrown from will be added to the stacktrace.
This is due to it being easier to see and a field that developers
are much less likely to override.
A nullable field `peer` has been added to `FlowException` and
`UnexpectedFlowEndException`. This is read later on (when peer info
is not available) to append the peer info to the stacktrace.

A test was failing due to `assumeFalse` (to stop a test running on windows)
being inside of a `assertThatExceptionOfType` block. The exception it
threw was consumed by the block instead of junit.
If a single whitelisted constraint is being used by input states and the version of the cordapp changes + is signed, then the constraint will transition to a signature constraint.
When a `UnexpectedFlowEndException` or a `FlowException` is received the
peer that the exception was thrown from will be added to the stacktrace.
This is due to it being easier to see and a field that developers
are much less likely to override.
A nullable field `peer` has been added to `FlowException` and
`UnexpectedFlowEndException`. This is read later on (when peer info
is not available) to append the peer info to the stacktrace.
`ExceptionSerialisingRpcOpsProxy` was removing information about the
original exception in dev mode. This code has been removed. Although
there is no check in `ExceptionSerialisingRpcOpsProxy` for dev mode
(and also due to it being about serialising),
`ExceptionMaskingRpcOpsProxy` will handle the removal of exception data
in non dev mode (production mode).

Normally, these requests are left unacknowledged in the MQ broker to allow recovery via installing the missing CorDapp. If this is not applicable then the request be dropped (and the other side informed of the error).
While the node is starting up, we now check for the presence of the certificates directory. This allows us to print out an easily understandable error message if the directory is not present. An exception is made for devMode, as devMode will result in the directory being created in any case.

If a hibernate error occurs (deadlock, for example) that would cause a flow to be sent to the hospital, hibernate logs the warnings and errors before we do. This results in duplication in the logs, and pollutes the log. To solve this, we create a new log appender named diagnostic-{node-name}.log and log any org.hibernate messages of warn and above to that file. This way, messages are not lost, which means that the information can be retrieved if need be.

Add a cache for notary identities in the PersistentIdentityService. This
solves a reported problem where notary identity lookup fails if its network
map entry is missing, which results in an exception when trying to insert
a state into the vault after recording a transaction.
Test if we have access to the logging path (baseDirectory/logs) before attempting to write to them. This allows us to shut down gracefully with an easily understandable error message.
Without doing this, the log4j2 will attempt to access the logPath when it first uses the logger in the call() function.

If a notary service is misconfigured with the type (either validating or not validating) stored in the node's configuration not matching that advertised in the network map cache, the notary will throw an exception and fail on startup.

There's a bug with the ServiceLoader which leaks a file handle to the app jar on shutdown. This causes an issue if a mock node is restarted in Windows. To avoid the problem completely we no longer overwrite any existing jars, as the jar to be copied will be same anyway.

TestCordapp has now two implementations to clearly separate the two use cases it has in the Corda repo:

The overload that takes in a bunch of the node parameters with default values has been fixed to the V3 version. New node parameters since V3 now only exist in NodeParameters.
The reason for this is otherwise each new release of Corda that introduces new node parameters will force a new startNode overload to be added to DriverDSL to preserve backwards compatibility.
NodeParameters has been moved to its own file and logLevel is removed as it doesn't do anything.

When it comes to KeyStores there are *2* passwords: 1 for the keyStore as a whole and separately there is one private keys within this keyStore.
Unfortunately, those 2 passwords have to be the same due to Artemis limitation, for more details please see:
`org.apache.activemq.artemis.core.remoting.impl.ssl.SSLSupport.loadKeyManagerFactory`
where it is calling `KeyManagerFactory.init()` with store password.
Before change in this PR, throughout our codebase there are multiple places where we assume that storePassword is the same as keyPassword, even in the classes that have nothing to do with Artemis.
This is of course less than ideal as TLS communication may be used not only for Artemis connectivity (e.g. Bridge/Float interaction in Ent) and it is unfair to impose same passwords constraint on that communication channel.
Therefore this PR is removing this limitation and properly separating storePassword from keyPassword.

created by a WireTransaction. This breaks the upgrade process if it was
in fact issued by a ContractUpgradeWireTransactions or a NotaryChangeWireTransaction.
The entry point to the API has been simplified to just requireing a list of packages to scan, with sensible defaults provided for the metadata. Because of the wither methods, having parameters for the metadata (with default values) seems unnecessary. Also the ability to scan just individual classes has been made internal, as it seems unlikely app developers would need that level of control when testing their apps.
TestCordappImpl is a data class and thus acts as a natural key for the Jar caching, where previously the key was the package names. This fixes an issue where it was not possible to create two CorDapp Jars of the same package but different metadata.

Internal tables (the tables from node and finance modules) are now tracked /created by Liquibase script.
Tables backing MappedSchemma in Cordapps are created by Hibernate (as before).
The PR scope added Liquibase library, setup code SchemaMigration and XML scripts and from Enterprise.
For existing database installation - the node will auto-upgrade to use Liquibase.
Method migrateOlderDatabaseToUseLiquibase checks for any 3.X existing Corda database to upgrade database to use Liquibase. When the existing database without Liquibase integral tables is detected, the node (at startup) will create Liquibase tracking tables and fill them with all migration scripts (marked as done), this ensure the database will look as it would use Liquibase from the beginning.
The database changes gradually introduced by the subsequent 3.X releases (3.1, 3.2) are conditionally run by Liquibase.

Moved start up logic of the various node components out of their c’tors and into “start” methods, which are called from Node.start(). The components themselves are created in the Node’s c’tor with minimal initialisation logic.
Certain things are not immediately available at construction time, which are instead given to the components at start time in an orderly fashion:

* The network parameters, and thus access to things like maxMessageSize and the contracts whitelist
* A running database - several components were doing database stuff their c’tors
* The node’s NodeInfo, and thus access to things like identities and addresses
The messaging service couldn’t be created in the Node’s c’tor due to initialisation issues with MockNode. This should be fixed in a later commit.
The loading of all node infos in the start method was unnecessary, both for the changePublisher and _loadDBSuccess, and the setting of _registrationFuture was incorrect.

They're prone to cause flaky tests due to the "allocated" ports already being taken by the system when eventually needed. Replaced usages with PortAllocation.Incremental.
Affected unit tests made into integration tests to avoid any issues in the future when unit tests are made to run in parallel.
Database table NODE_ATTACHMENTS_CONTRACT_CLASS_NAME in v3.0 was changed to NODE_ATTCHMENTS_CONTRACTS in v3.1 and then finally NODE_ATTACHMENTS_CONTRACTS on current master. Users may omit the upgrade note and run into errors. After the change the node will not start if the new table name is not found and any other older ones is found.

Minor: TimedFlowTests are sometimes flaky in CI since the flow result future completes before the progress tracker is set to DONE. Instead of checking the progress tracker step immediately, wait for it with explicit timeout.
We parse command line arguments twice, once in `CordaCaplet.java`
to determine the config file and base directory, and then again
in NodeArgsParser. NodeArgsParser accepts long options with a
single hyphen, while CordaCaplet did not. This changes
CordaCaplet to accept `--config-file` as well as `-config-file`,
same for base-dir.

This requires the class carpenter to be able to run in a "lenient" mode where it permits synthesised classes to implement interfaces with unimplemented methods.
The list of CorDapps jars is no longer passed in via the cmd line but is now expected to be placed in the bootstrapped directory.
Ended up being a bit of a refactor to cater for unit testing, and also tidied up the bootstrapper docs.
* CORDA-1645: Checkpoint before calling an idempotent sub-flow.
When an idempotent sub-flow causes a flow restart, the flow will be
replayed from the last checkpoint before the sub-flow invocation.
However, the logic between the last checkpoint and the sub-flow invocation
may contain side-effects which shouldn't be replayed. Thus we need to persist
a checkpoint just before an idempotent sub-flow is invoked.
This will ensure that the notary client flow will retry over a sufficient
period of time for the notary to update its network map.
With a backoff base of 1.8 and 5 retries the last retry will fire after
about 20 min 8 sec of the initial flow start:

As reported in [CORDA-1609](https://r3-cev.atlassian.net/browse/CORDA-1609),
`CordaRPCClientConfiguration.default` is not accessible from Java since
`default` is a reserved keyword.
As part of the refactor made in #2831, `CordaRPCClientConfiguration` went
from being a data class to an interface with a backing implementation of
type `CordaRPCClientConfigurationImpl`.
This resulted in Java users having to rewrite code that was on the form:

final CordaRPCClient client = new CordaRPCClient(
nodeAddress, CordaRPCClientConfiguration.DEFAULT
);
```
to something like this:
```java
final CordaRPCClient client = new CordaRPCClient(
nodeAddress, CordaRPCClientConfiguration.Companion.default()
);
```
However, this does not work. The user would get a compilation error because
`default` is a reserved keyword in Java.
Since `CordaRPCClientConfiguration` has been made an interface, there is no
easy way of introducing a static final field on the interface from Kotlin.
Consequently, I've changed this back to using a `class` with a static field
named `DEFAULT` instead of the static method `default()`.
It should be noted that `default()` / `DEFAULT` is currently only used
internally to pass in default values in `CordaRPCClient.kt` and
`CordaRPCClientUtils.kt`. That said, it is exposed as part of our API
surface and consequently shouldn't be broken.
The latter means that in the above example, the user would actually not
have to provide the parameter at all:

```
As can be seen from the definition of `CordaRPCClient`:

this change can be applied without affecting our commitment to a
backwards compatible API..

At startup node warns about any MappedSchema containing a JPA entity referencing to another JPA entity from a different MappedSchema. Cross reference between JPA entities across different MappedSchema objects can cause operational issues while evolving one MappedSchema object or migrating it's data. Doc API: Persistence documentation no longer suggests mapping between different schemas.
It will re-run automatically from last checkpoint on node restart, allowing the opportunity to resolve the issue, something required when dealing with contract constraint failures.
* CORDA-1589 Flow hospital reports incorrect number of patients and fix flakey associated test.
* Compare before and after count, not against zero. There's a leak in killFlow that will be easier to address in or after Shams PR.

The original message-based retry approach does not work well with the new
flow state machine due to the way sessions are handled. We decided to move
the retry logic to flow-level: introduce RetryableFlow that won't have
checkpoints persisted and will be restarted after a configurable timeout
if it does not complete in time.
The RetryableFlow functionality will be internal for now, as it's mainly
tailored for the notary client flow, and there are many subtle ways it can
fail when used with arbitrary flows.

JPA/Hibernate entities need to impose the correct NULL/NOT NULL constraints on the database - whatever these correct values actually are.
API change: net.corda.core.schemas.PersistentStateRef fields (index and txId) are now non-nullable. Rationale: The fields were always effectively non-nullable - values were set from non-nullable fields of other objects. The class is used in context of database table Primary Key of for other entities and a database already imposes those columns as non-nullable (even if JPA annotation nullable=false was absent).
