refactor: flatten pages structure and update setup.php for Git Sync

- Move pages/{fqdn}/pages/* to pages/{fqdn}/*
- Update setup.php to look at /user/pages/{hostname}
- Prepare for Git Sync to sync pages/ to user/pages/
