docs: reachableceo wording sweep + provisioning status

Direct order 2026-09-09: name the principal (reachableceo), never 'the
founder'. prompt.md left verbatim as a historical artifact. org-chart
rows updated for the 2026-09-09 provisioning completion (#942).
