The ALARM_STRUCT() and DECLARE_TRIGGER() macros included terminating
semicolons, but their invocation should always be followed by a
semicolon, otherwise etags(1) gets confused, so their included semicolon
was redundant.

Instead of int64_t. Fixes some -Wsign-compare warnings.
Replace sqlite_exec_int64() with sqlite_exec_uint64().
Also store rowid as uint64_t, and use 0 not -1 to indicate

New UNUSED(param) macro suppresses the warning for a specific parameter
