invert npmignore to include what we want (#4605)

* invert npmignore to include what we want
* remove line for no-longer-existent src/**/*.spec.js files from npmignore
* cleanup npmignore comments
* remove platform dirs from npmignore, they were removed from the repo
* remove one more platform reference from npmignore
* publish example/ and app.js, at least for now
