Norman Feske cda07b7da0 base: simplification of the IPC code

This commit replaces the stateful 'Ipc_client' type with the plain
function 'ipc_call' that takes all the needed state as arguments.
The stateful 'Ipc_server' class is retained but it moved from the public
API to the internal ipc_server.h header. The kernel-specific
implementations were cleaned up and simplified. E.g., the 'wait'
function does no longer exist. The badge and exception code are no
longer carried in the message buffers but are handled in kernel-specific
ways.

This is generic part of the Genode implementation. It consists of two parts:

:_Core_: is the ultimate root of the Genode application tree
  and provides abstractions for the lowest-level hardware resources
  such as RAM, ROM, CPU, and generic device access. All generic parts of Core
  can be found here - for system-specific implementations refer to the
  appropriate 'base-<system>' directory.

:_Base libraries and protocols_: that are used by each Genode component
  to interact with other components. This is the glue that holds everything
  together.
