kernel: Update versions. Remove older kernels

In this update the following kernels updated:
 * 4.0.3 -> 4.0.4
 * 3.18.13 -> 3.18.14
 * 3.14.42 -> 3.14.43
 * 3.12.42 -> 3.12.43
 * 3.10.78 -> 3.10.79
 * 3.2.68 -> 3.2.69
 * 2.6.32.65 -> 3.6.32.66

Also, let's remove the older EOL'd kernels.
This makes updating kernel versions easier.
